Cosmetic Model Terms and Conditions:
Please find below the Canadian Board of Aesthetic Medicine (hereinafter as “CBAM”) terms and conditions related to the Models attending the Cosmetic Courses. If you have any questions, please do not hesitate to contact us.
Much effort has been taken to ensure the safety and advisability of the information presented and applied in all the cosmetic and aesthetic courses presented by the Canadian Board of Aesthetic Medicine (CBAM). All procedures are taught within the standards set out by the College of Physicians and Surgeons of Ontario and the College of Nurses of Ontario. With any medical procedure, there is always a risk of injury when performing any type of intervention. The risk of injury increases when a procedure is performed without judicious precautions, negligence on the providers behalf, and/or when the recipient of the intervention has unique biological or other factors that make them more vulnerable to negative reactions. Additionally, it is possible for clients to unexpectedly have an adverse reaction where their overall health becomes compromised during or after completion of their treatment.
By agreeing to be a model for the CBAM course, you certify to the following attestation:
You, as a cosmetic model, are totally aware that you are in a training facility and the trainees are health care professionals or health care professional students who are supervised by a qualified instructor(s) during the interview, assessment and injection (treatment). As these are training courses, every trainee will train at a different speed. This means that the waiting times for you as a model can be slightly longer than that of a clinic appointment.
Due the clinical environment, we strongly advise you not to bring children to your appointment. We understand you may want to bring someone else with you to your appointment but we do ask that this is kept to a minimum. If you do bring someone with you, please note they are not allowed to go into the treatment room with you. We would also ask patients to respect the privacy of others during the course of treatment.
Due to the nature of training courses, on occasion we may need to cancel or change your appointment. You will be advised of this and an alternative appointment will be offered on another day/course. If the cancellation is initiated by the CBAM, you are entitled to receive the full refund of your paid deposit, if you do not wish to change the date of your appointment to another date.
Prior to treatment, you will be verbally discussed any risks, alternative treatments, and adverse effects which can potentially occur from treatment with the injector and/or supervisor at the course. By agreeing to this form, you are confirming you are aware of the risks and benefits, potential contraindications, and adverse effects which can arise from treatment. Please be aware if these instances do occur, the Canadian board of Medical Aesthetics, the staff, or instructors do have the facilities to treat and deal with adverse effects but do not take legal or personal liability.
You hereby release Canadian Board of Aesthetic Medicine and other associated instructors, faculties and staff from any and all liability of any kind, directly or indirectly related to your treatment.
You agree that all videos and images taken in courses (which you can be in any of the images or videos) can be used or shared by the Canadian Board of Aesthetic Medicine for educational and marketing purposes for the benefit of other healthcare practitioners in the field. Should you not want to share your images or videos, please mention this to the staff on the day of the course.
Before & After Photos:
We are required to take “before and after photos” of your treatment to keep on your file. You have complete control over what happens to these photos. We will not share these photos with any third party without your consent.
On booking your cosmetic model appointment we require you to pay a c$100 deposit. This amount will be deducted from the total cost of the treatment. The total cost of the treatment must be paid on the day of the treatment by credit card or cash. To see the fees, please refer to the email has been sent to you by CBAM. Any payments made for the goods and services provided to the models are non-refundable.
If you cancel your appointment at least 5 days before the course date, we can transfer your deposit to another appointment or give you a full refund. If you cancel your appointment within 5 days of the course date, you will not be entitled to a refund. If you do not attend an appointment your deposit will be lost.
You must provide accurate up-to-date information including your medical history on each of your appointments. We will not be held responsible for any incorrect information given.
Follow up Appointments/Treatments:
If you feel you need a follow up treatment or visit for any reason please contact the CBAM at 1-866-430-8677 or email us at info@CBAMedicine.com.
In most cases, we will recommend you to let the results of your treatment to settle for about 2 weeks and we will arrange a follow up appointment with you between 2 and 3 weeks after the treatment. In some cases, the follow up appointment may not in the original place of treatment and you may need to travel to a different clinic in where your direct supervisor physician practices for the follow up appointment.
You will receive the instructions regarding the follow up appointments on your initial appointment session.
